I would like to re-instate "Adverse drug events" as a separate topic or to redefine "Adverse drug reaction" with an expanded definition similar to ADE. The "Adverse drug events" (ADE) currently redirects to "Adverse drug reaction" (ADR). The ADR has been defined as "a side effect at normal dosage". An ADE in Australian usage includes incorrect prescribing, transcription errors, dispensing errors, known side effects and other reasons.
