Ishii is of mixed Japanese, Chinese and American heritage. After watching her parents being brutally murdered by a Yakuza crime lord, she spends years training herself to be a ruthless killing machine so that she could get revenge; she finally gets it after sneaking into the yakuza lord's house posing as a child prostitute and stabbing him to death.

Formerly in the employ of Bill and a member of The Deadly Viper Assassination Squad, along with the Bride, she participated in the attack on the Bride's wedding, which leads to the Bride seeking her out for revenge when she awakes from a coma. By then, Ishii had become the head of the Yakuza, over the objections of its founding members, who looked down upon her mixed racial makeup (she stifled such criticism by personally killing anyone who brought it up.) She also has her own personal army of sword-wielding, masked punks known as the Crazy 88. She rules over her criminal empire with the help of her lawyer, second lieutenant, and best friend, Sophie Fatale.

The Bride confronts the gang one night inside a restauraunt, where she annihilates all of Ishii's underlings, severely maims Fatale, and eventually slays Ishii herself in a duel, completing the first target on her hit list.
